Lost information when somebody clicks the link and disappear the information

Hi , I found that I lost email requesters here and pop up email address here . This is not what I wish. I have multiples email address connected to each other . When I click the link it disappears the first information provided. 

Please assist to check on this . Form : Work Order Request 

Could you please explain the issue in details? Also, please provide us the link you click so we can test it on our end.
