These Embed options are only available for Card forms. It is not available for Classic forms.
Please do let us know if you have any other queries.

Can you please check why my form is not working

Forms which has single question in one page are said to be Card forms. The form which you are using now is Classic form (all questions in one page).
To embed the form in your web page, go to PUBLISH > EMBED. And copy the code, add it to the web page where you want to display the form.
Use this guide to know more about adding a form to web page.

Thanks for the reply i have used the embed code to be added to the page

Here is the problem
If we access the form directly then it works
But if we click on the website link to go to the form then form is not loading first time, if we refresh the page then it is loading
