This has been an ongoing problem that I am told it has to do with my email address and has been corrected each time. Then it happens again in about a month. Is there any way to make sure the corrections keep working?

Please take note of the reason, your mail service provider has rejected the emails coming from Jotform. Check this guide for the solutions and further informations: How to setup email alerts to prevent "emails not being received" issues.
Also, if you have the option to add email@example.com to your Safe Sender list then please try doing so.
I have now removed your email address from the said list so it should be working fine now.
Furthermore, we are having problems with comcast.net emails but we will try contact them about the issue and let you know for the results once done.
