Is this possible to achieve with form calculation?

    I have read all of the documentation on forms calculations and haven't been able to find what I want. I have attached a file that my client gave me. Is this possible with JotForm? Also, is it possible to remove the JotForm branding at the bottom of our forms? Their site is not live yet, so I can't provide a URL.

    Yes, that is possible. You can also customize your form using custom CSS Code to meet your client's requirements.

    Once you finish the form and need additional help, you can post your question here and we will help you to solve the problem.

    For jotform branding, you can remove it by unchecking (Show "Powered by Jotform" on my forms) in your account settings: https://www.jotform.com/myaccount/settings.

    Thank you. The video link that you sent for calculations is the same one that I watched before I posted my question, because it was so different than what I was looking for. I need to know how to make the calculations in the image that I sent you and that video doesn't address that.

    Sorry for delayed response. Could you please check this demo form: https://form.jotform.com/90218602972962. What is "Average" for? Could you give us a computation for that so we could add it to the demo form?

    You can clone the form to know how it was done: How to Clone an Existing Form from a URL.

    Also, please tell us if there is a mistake in the calculation so we can fix it.

    Thank you, I'll take a look and get back to you shortly. Also, I have been
    waiting over 24 for an answer to a HIPAA question. Is that something you
    could help me with? I have an urgent need that must be dealt with today. I
    might as well ask you in case you can help me. I have a HIPAA compliant
    account for one doctor client. But all our other clients don't need it. I
    can't log in right now because I'm getting a "temporarily unavailable"
    message at hipaa.jotform.com. And I don't want the HIPAA compliant icon
    showing up at the bottom of all of our forms, only the doctor's forms. I
    don't want to pay for two accounts.

    This is perfect!! The "average" just referred to what their average hourly
    charge is when doing this process with a car. So that figure is up to the
    person filling out the form. I tried it with an hourly rate of $50/hour and
    it worked. So the only thing we would need to change is the price, because
    right now it is in Pounds Sterling and it needs to be changed to USD. Is
    that something we will have control over? And will we will be able to
    format the form to look like their sample with the words on the left and
    the fields on the right?

    Yes, it is possible to add the USD symbol in the widget itself.

    Yes, it is possible using the Form Designer. 

    1. Change the label alignment of the text entry fields to "Left".

    2. Increase the form width from the Form Designer.

    3. In the Advanced Designer, you can change the spacing between the label and the field by changing the "Label spacing", under the Design tab.

    I am meeting with my client today to show them this demo form that you set up. Is there a way for that demo form to show up in my list of forms in my account? Or do I need to recreate it? I would prefer to use yours and just make the styling edits that you gave instructions for. But I don't know where to find the actual form.

    You can clone the form URL that my colleague has provided above into your account and then make the necessary edits. You need not recreate the entire form.
