    Hi Rocket15

When a header field is added to a form, whatever you edit its text to state will be reflected in the name of the form. But you can also directly click "Title Me" (in other words, "Give Me a Title" or "Give Me a Name") in the orange title bar at the top of the form builder and put the name of your choice.

Navigation: My Forms > Click form you wish to name > Click "Edit" > Click "Title Me" > Edit the title > Save change.
