Não consigo receber o formulário em meu e-mail.

Bom dia!

Criei um formulário com o titulo "Orçamento". Preencho todas as respostas do formulário e envio, mas não recebo em meu e-mail ele preenchido. O que faço?

Recebo essas informações em meu e-mail:

Nome {empresa} 
Sobrenome {nome} 
Telefone {fccCall} 
E-mail {email13} 
Como conheceu o site: {comoConheceu} 
Itens do pedido {selecioneOs}

Your Email Notification table was customized and the field questions do not match with the field values. Only the value of the email field is correct. 

I would suggest you to delete the existing Email Notification and create a new one. New Notification will have all field questions and values.
