The upload photo button on the form won't work

and I've tried everything - we are in a crucial part of casting and this is throwing everything out the window... we've already lost close to $5000 over the last 24 hours 

I checked your form and the file upload button is functional on my end:

Is the form embedded to a web page or is there a specific way to reproduce the issue?  As far as I can tell, the form should be allowing for file uploads.
