Upon checking, I can only see this form - https://www.jotformeu.com/form/90227360903351, do you have other forms besides this?

I also checked your account history logs but there is no deletion of form. Please tell us the name of the form or URL form you are referring to so we can check it. You are probably referring the form to your other account (if you have another account).
