Would like to prepopulate certain fields based on contractor selection

Also would like to prepopulate certain fields based on that contractor selection (i.e. email, address, location, etc.)

It is possible to prepopulate the fields based on the unique codes (e.g. contractor's code) with a Spreadsheet to form widget.
