retrieve username of account

Hi JotForm Support,

I am an employee of Revision Skincare LLC, we have used JotForm in one of our projects, the consultant who worked on that project is no more with us. 

We need to access the JotForm account to work with the forms but we do not have the credentials with us.

Can someone please assist us figure this out?
