I am not able to login to my account

    Whenever I try to login to my jotform account, I keep the getting the message Login-suspended.

    Can you assist me in removing the suspension.

    Upon review of your account it was suspended due to forms: http://www.jotform.me/form/90262884882468http://www.jotform.me/form/90262666001449 and http://www.jotform.me/form/90263057072452 are requesting login credentials.

    Kindly provide us these proofs :

    1. The URL where your forms are used.

    2. Your website or proof of affiliation to a legitimate institution. We also need you to change your registered email to a corporate one.

    3. Share with us documents that will prove that you're authorized to use the said logos.

    Send all of these to reviewers@jotform.com with Thread #1716284 as your subject. We will review it and get back to you about the status of your accounts.

    Or, we can just remove the logos and lift the suspension on your account.

    We would appreciate your cooperation on this matter.
