Okay, I was able to restore the form myself, but I have another question:
How can I edit my form so when I receive an email from the form and hit Reply, the sender's email is in the "To" field by default. Currently, my email ends up in the "From" field, I end up replying to myself, and then I have to go back and manually cut and paste the sender's email into the "To" box to reply to their email. -

Glad that you've figured it out.
For Squarespace, we recommend the iframe method as per our guide here https://www.jotform.com/help/37-Adding-a-form-to-Squarespace.
For your second question, I will have that answered on a new thread here https://www.jotform.com/answers/1716744.
