Help create build a from for car classified site

Hi

    I have been everywhere to find my needs to no avail,  you are closest, but i need help. Wordpress does not do anything close.

I am building a car classifieds site and i need a form which shows all of the cars attributes with a box on the right hand side for the advertisers answers. I will then have to integrate it into my site ,,,can you help.there will be a vertical line of words such as 

CAR

MILEAGE

TRIM

ETC

ETC

ETC

DESCRIPTION

And over on the right hand side will be a box for the advertiser to fill in. At the bottom will be a larger box named description that will be filled with text of general info.

I cannot find anything close and i wonder if you can tell me how to build this form.

At the bottom there is also contact, phone, email  etc etc etc

To clarify, would users select a car and the car attributes would show?
