Pre Sales Questions on HIPAA forms

    I have some questions of the product before purchasing the service. This would be for the HIPAA compliant web forms. 

    BAA wise, is this company based out of the United States? Can we make it closed domains so that the only people that can get form submission data is sent to our set domains? When a form is submitted, I believe we have an option of either logging into the website or getting a password protected email? Is the password set one time and used each time a form is submitted? Just to confirm, we can embed the form to our website correct?

    Submissions can be reviewed at your JotForm account and you also can receive email notifications containing some information. But, there is an option to include PDF files of submissions attached to emails. PDF files will be password protected, password is set up form wide (same password for every submission): How-to-include-submissions-as-PDF-attachment-in-the-email-notifications
