Prior employee setup a Jotform...how do I access account?

Do you have the username or links to forms that are hosted in this account? Is there a way for you to contact this person to ask him for the login credentials? Please note that only the owner of the account can give you access, or transfer forms ownership to another account.

We have found a paid account under an email address with the domain "@genomichealth.com". The username is "OncotypeARV7", one of the forms is: http://www.jotform.us/form/83596596058172
