When I've tested the form by providing the link via an email message there is a delay before the form eventually loads.

Your form seems to load fine on my end. The problem might be related to our security layer, CloudFlare. Would you please try to change the .us to .com on your form URL? Example: http://www.jotform.com/form/123456789, Or kindly try to clear your browser's cache and cookies see if that makes differences.
