How can I create a questionnaire?

Regarding your question specifically:
1) Let's say you have radio buttons (for yes and no answers):
2) And add one text field that will be the additional explanation field:
3) Finally, add a condition that will show your text field when yes is selected:Guide: How to Show or Hide Fields Base on User's Answer

JudeCReplied on February 6, 2019 at 5:43 AMThanks but I just want a form where I ask questions and people can write their answers. It’s not a yes/no or multiple choice situation. I’ve tried Google forms and that is useless. Just don’t know how people do these things. Sorry but I just get so tired of all these 30 minute jobs turning into a 3 hour marathon with no tangible result.
