Request to Move Form to a Different Account

We are in the process of consolidating accounts and I need assistance moving a form to a different account. 

This form is under the account jahrens@lunagrill.com and it titled "Clone of Luna Grill Systems User Form ( https://www.jotform.com/build/90064924984163 )

We need it transferred to the account under forms@lunagrill.com
