Questions coming back out of order

When our customers submit their Accountability Attachment 6.4 b Interventions Reporting their answers are coming back out of order, they can not change that on their end, why is this happening?

What do you mean by out of order? Do you mean the fields in the email are not on their correct order based on how you added them in the form builder? If so, your form is too long, so I suggest deleting the current email template and create a new one.
