Voice Recorder widget is not showing up in my webpage.

I have a form that has a record option in it. When I copy the code to my godaddy gocentral website, I see the form and the submit option but the form does not have the record option. You will need to scroll down to see the form. I have them leave a voicemail since the form is not working properly. 

Please remove it and use another embedding method, I would suggest you to try with the Iframe code: https://www.jotform.com/help/148-Getting-the-Form-iFrame-Code
