How can I have the submitted form show (and print) the whole form not just the answers?

I've added a waiver that I would like to be able to print after someone has submitted the form. The signature prints fine, but all the questions and text on the form don't print. Is there a way to have the entire form print?

You can get the PDF copy of the submitted form as an attachment in the email notification. To enable this, please follow the steps below:

Go to Settings >> Emails >> Email Notification >> Edit

Go to PDF Attachment and enable the same.
