We are using your form service for our returns on our eccomerce website and we want some fields that are just to be entered internally once the completed form has been sent to us.
But if they do not enter details into a certain field then the email notification does not show those fields. Please see http://www.cidproducts.co.uk/return-request-form.html to see what I mean.
Even better It would be good if we could hide these questions from the website and only print them in the email notification to us.

I have made a clone of your form as part of a solution for you:
You can clone to see the changes I made. At the bottom, I added a radio button to allow the option to show the Office section only when needed.
I also made this video to show you how you can use the submissions page in order to add the necessary comments on the office section:
That is one option for you.
As per the other option of only printing these fields on your notification, Here is a clone of your forms. I have deleted the fields that were for Office Use only.
I also edited the Notification. I manually added the fields that you had for office use onto the body of the notification:
