I am showing locked out of my account due to a possible phishing attempt. We created an online credit card authorization form for our dealers to be able to enter their information and it be sent to us. Our business keeps credit cards on file, not paypal, square, etc. (which are the options given). We primarily use jotform for our order forms though so I need those to be up and running because my business has come to a halt.

Upon review of your account it was found your credit card collection form is the one considered infringing our Terms of Use, since no part of the credit card number, expiration date or security codes are allowed on plain text fields.
If you would allow us to delete the fields in order to be able to re-activate your account, we will proceed to do so.
We will wait for your answer.

Upon checking, your account is already active and your form has now been deleted. You can now log in.

Upon review of your account it was found suspended due to form: http://www.jotform.us/form/90284849335163 requesting credit card information on plain field texts.
Due to our PCI certification, we are required to deny the collection of credit card details on regular forms.
However, we have other PCI-compliant ways to do this. All you need to do is choose any of the payment processors available in our platform in order to integrate in your forms. You still won't be able to collect credit card details directly though.
If you only want to collect your customer's payment details and charge them at a later date, you can take advantage of a feature called "Payment Authorization". To learn more, kindly follow this guide on How to Enable Payment Authorization.
The infringing fields containing credit card information has been deleted and your account has been reactivated.
