On behalf of Canadian Quilters' Association, Ian Thompson set up a jotform account in 2016. His email is ianthocan@gmail.com
As his tenure ended with our association at the end of 2017 we moved his information to a new account called CQAregistration and cancelled any connect with the former account.
I was informed by him that his credit card was charged for the original account in the amount of $217.28
I no longer have access to the form and forget the name even as we use a completely new form.
How do we get his credit card charge reversed and delete the unrequired form.

I have checked the email address you have shared but I can't seem to pull any account under it.
We may need the transaction details to track the account.
Please provide us with the following details:
1)Name on the Card.
2)Type of credit card.
3) Last 4 digits of the used credit card.
4) Expiration date of the credit card.
5) Date when the charge occurred.
6) Amount charged.We will wait for your response.
