How can I make a command to insert a new field?

I would like a button to create a copy of page 2 on my first app. 

i.e. "Add another issue" on a button that brings up a copy of page 2, to enter in a second and further issue after the first one. 

We have a widget called the Configurable List widget, which dynamically adds a set of fields, however, it does not support File Upload fields: https://www.jotform.com/help/282-How-to-Set-Up-the-Configurable-List-Widget 

So, your other option would consist in setting a limit of issues(set of fields) that the user can add. Check the following example where the user can add up to 3:

1) Add a Divider field at the top of each set of fields:

2) Remove the "Issue Number" field, and use a Spinner field instead, where you have to limit the entry to 3:
