For my Client Referral form, how do I run a report for a date range?

Client Referral form template - how do I run a report for a specific date range?

Why did the format change?  Is it because this is a PDF Editor form?

It gives me options for Excel, Grid CSV, etc. but not set up like old reports to choose dates  -  

I  did  check your form and you seems to be using  old version of submission page. You can select  the date  range in your form's submission page. Please check the screenshot below:
