I have been trying to get this figured out since Sunday/Monday of this week.
I created two forms and I want to get this fixed so I can let my clients know about my services/packages.
The first form I created was this one --- http://form.jotform.us/form/30262552701141
and it was for the URL here: http://simplyroyalty.com/shop/
The second form I created was this one --- http://www.jotform.com/?formID=30286230863149
and it was for the URL here: http://simplyroyalty.com/coffee-chat-curiosity/
Please help me, I've contact your assistance support via email and I was referred to this article: http://www.jotform.com/help/4-Adding-Forms-to-Wordpress --- but I'm still stuck! :(

I've contacted both my server (Bluehost) & domain registar (name cheap) and they've cleared it both on their ends. Also, I've reset my cache multiple times.Page URL:

The reason why this is not working is because form being served on .us domains have been affected by an issue , that is explained here
At any rate , if you are not an AT&T user , do the following
for your page http://simplyroyalty.com/coffee-chat-curiosity/
Please change it to this one
Do the same with others if you ever see the .us for the domain
