Log into JotForm account

    Hello - 

    Our organization created a Jot Form and cannot access it since the employee who created it, is no longer with the org. We simply need to update the contact info.

    *** Page needs to link to vsoldati@ovarian.org and 215 997 8075 instead of ksilverman@ovarian.org


    If reset instructions can be sent to myself (vsoldati@ovarian.org) or please let me know how to do this..thank you so much!

    You can reset the log in password of the account using the 'Forgot Password' link in the log in page https://www.jotform.com/login

    The reset instructions will be sent to the email ksilverman@ovarian.org

    The email no longer exists for ksilverman@ovarian.org which is the issue.

    Upon checking ksilverman@ovarian.org account, I can see that it only has one form saved in the account. If you don't have access to the email, you can create a new account and clone the existing form by following this guide https://www.jotform.com/help/42-How-to-Clone-an-Existing-Form-from-a-URL.

    Unfortunately, we cannot disable the form without getting the request from the form's owner or getting a proof that the account belongs to you, note that this is due to security reasons. 

    I would recommend you to replace the form URL on all the places you use it in order to have the submissions received on the new form.
