Hello,
We currently have an electronic form embedded on our website and being sent in a reminder text message. When the forms are submitted, one of the questions that the client is answering is showing " Type a Question" instead of what the question on the form is. Is there a way that this can be corrected?

I have cloned your form and tried to replicate the issue and I was able to replicate the same.
There are actually 2 fields where you will find "Type a question"
Upon investigation I found the below error:
When you will check under Settings >> Emails >> edit Email Notification
Under Email Content field change the name of the below-highlighted fields:
Please refer to the screencast below about how to change the name of the fields (I have shown the field names highlighted in black):
Once you save this, you will see a question in place of "Type a question" in the submitted responses.
