How can I load a contract from my documents on my computer?

I have a newborn session contract I would like to use as a form but how do I upload it? Thank you!

Well, it's possible to embed a PDF in the online form but it can not be signed or cannot be submitted even if you fill it out. However, if you want, you can create an online form similar to your PDF and then generate a PDF report.

You can download, print, customize the PDF report to your preference.

You can also include them in your notification and autoresponder so once the form is submitted both you and your users will receive a PDF with the email.
