    Is there a way of adding pictures? I need to make an image survey with suggestions on things to donate.

    It would have the picture of the item then 3 boxes- ( Yes, donate; Keep; Other with option to type suggestions)

    Yes, it should be possible. For example, add an Image element, and then a Single Choice field below.
