Account was auto suspended?

    Good afternoon, it seems that my account was autosuspended this afternoon. I was working on a mock up of a member registration form and I'm guessing your system saw that as being suspicious. Can you please let me know how I can have my account unlocked? Also, any guidelines on how to avoid this in the future would be great.

    Upon review of your account it was found automatically suspended (autosuspended) by our Phishing Detector Tool due to form: http://www.jotform.us/form/90425880460154 requesting login credentials (Username/Password)

    Gathering this type of data infringes our Terms of Use.

    Asking for an access code to conditionally show/hide fields within your form is okay. However, you should refrain from using "Password" as your label to avoid being flagged by our phishing detector. Kindly change it to something similar like an "access code". Other than that, no other kinds of passwords or user credentials are allowed to be collected.

    The infringing field has been deleted and your account has been reactivated, refrain from creating login forms because it can trigger an automatically suspension by our Phishing Detector Tool.
