    Hi Bazil,

Do you want to to link the QR code to the form or embed a QR code in the form?

If you just want to link to it then all you need to do is to use the direct link to the form when building your QR code.  So, make a QR using some online tool (eg http://qrcode.kaywa.com/) and for the address just put the jotform address.  Lets says it's form ID 10724236262, then you would link to:

    http://www.jotform.com/10724236262

If you want to embed the code in the form, just treat it like a regular image and insert it in the form builder.

    Thanks,

    Daniel
