This happened to me once. just clone that form, and use the new link. just a glitch.

You should always see the submissions in your Forms Submissions page eventhough our emails or Jotform emails are rejected sometimes by the other service provider. I've check your form and I can see one form that has six recent submissions.
I have just cleared your forms cache, I hope this helps. Please kindly share with us the webpage URL where the form is embedded so we can check.
