Need to find out what my username is

I am the owner of www.thetexasrainman.com. We've been using Jotform for several years. The person I had set it up is no longer with our company and I need to know what the username and email associated with my account is. 

I am not sure if the code below will help you locate it. If I just knew what email address is needed to change the password, I am sure I can access the email.

The forms provided by you are associated with the account thetexasrainman registered using your email address. If you do not have the password, you may request for a password reset from the login screen so that the password reset link will be sent to the account registered email address.
