Hi there,
I am looking to create a Jotform Patient Assessment form for my new business.
I would rather the form is not online via my website, as this is too public facing.
Instead, I'd like to be able to send a form to clients to their inbox, then have them submit a response. Can this be done (via a Jotform link?) or is there alternative solution?
Also when a form is sent back and appears in my Jotform account inbox, is there a print option to make a paper-copy?

Hello Mark, JotForm should meet your requirements.
Unfortunately, we do not have an option to make a private form so it could not appear on search results, but as long as you do not share the link on public (forums, web pages etc), search engines can not capture it. So, you can create an online form and share it via email only: How-to-Send-a-Form-via-Email
The other option would be to create a fillable PDF form. This type of forms can be submitted using Adobe PDF viewer or Foxit PDF viewer. However, fillable PDF forms do not support online payments and most of the widgets. Submissions would be received to your JotForm account, where you can manage, download or print them: How-to-View-Form-Submissions
