Dear JotForm employee,
I am having trouble receiving notification emails for my form I created. I verified the recipient of the submission notification and the test email works as intended, I also applied a spam filter to the gmail address that is the intended email recipient. Furthermore, this email is not on the bounce list. I am still not receiving notification emails when a form is submitted from a separate email address. The code on the website in which the form is used is also all up to date, how can I fix this?

I am sorry for the trouble caused to you. I did check your last edited form "Appointment Request Form" and it seems you have only added auto-responder email alert which will send submission email to the user who submitted form. Please check the screenshot below:
If you want to send submission email to one of your own email address, I would suggest you to please add a notification email alert in form and set the recipient email to desired email address. I did test you form with my email address and I did receive auto-responder email successfully. Please check the screenshot below:
