I cannot access the documents in the form

Are you referring to your Service Attendance Report Form? It is the only form you have with file uploads. You are accepting image uploads, and I tried downloading three (3) files, and they all worked for me. 

Your File Privacy setting in the account is not enabled, so this shouldn't be the cause. Would you mind sharing us the file that you cannot access? We'll check that to our end.
