I can not access my account

    Hello! I can not access my account, it always appears with the message "Login failed - AUTOSUSPENDED". How can we solve this problem? My user: adbotafogose

    Upon review of your account it was found automatically suspended (autosuspended) due to form: http://www.jotformz.com/form/90503496228661  requesting login credentials (Username and Password)

    Gathering this type of data violates our Terms of Use.

    Asking for an access code to conditionally show/hide fields within your form is okay. However, you should refrain from using "Senha" as your label to avoid being flagged by our phishing detector. Kindly change it to something similar like an "access code". Other than that, no other kinds of passwords or user credentials are allowed to be collected.
