How to embed Fillable PDF and send notification email?

I want to embed a fillable pdf to my website.

Here's what I want to happen:

1) User visits website and fills out fillable app

2) User hits submit button

3) Form and all it's content is emailed to me

Possible? If so, how?

Do note that you cannot embed Fillable PDF, you may however, share the URL link or download.

If you have setup notification emails with your form, you will receive an email when the form is submitted.
