Can the attached screenshot be successfully created into a form.

Need the reports to generate something like this.

I attempted a form but having trouble with adding another field, like McDonald's with its own set of items.  Then subtotal then a grand total. 

Also, excel works great but lumps all items in one cell, I need them in separate cells.

In order to show the data in different cells, you would need to collect them with separate fields. It is possible to re-create products and set up total calculation on a form. Please refer to this guide: How-to-Perform-Form-Calculation-Using-a-Widget
