Additional:(that's my personal Facebook Account. I can't log into my account. It gives me lots of hurt. Please response me as early as possible.

I am not sure if I have understood your question correctly. Do you mean to say that your Jotform account is disable or your Facebook account is disabled?

I am unable to find any details with your email address helpmefb****@gmail.com. Please let us know any of your form ID, form URL or webpage URL where you have embedded your form and we will take a look.

If the issue is related to your Facebook account, I would suggest you to please connect with FB support.
