Form url is http://form.jotformpro.com/jsform/30345258193959
The fields "Nominee's Name" and "Years Of Youth Service" have their labels selected to align at the top. Yet, when embedded at www.georgianazarene.org/timothyaward1.html, they display as left aligned.
Using an iFrame displays them correctly, but also leaves that horribly ugly mountain of space below the submit button, as shown at www.georgianazarene.org/timothyaward.html.
So, I suppose I either need to know how to get the label alignments to display correctly upon embedding, or how to eliminate the excessive space below the submit button on upon using an iFrame.

I have cleared the cache for your forms on our side. Here is what I see on your site now:
Please let us know if that has solved the problem on your end as well!
