It will help us provide you with the appropriate response and suggestions if you provide more and clearer details of the issue.

I checked the form Conditions and I see there were existing saved conditions on the form already.

Please provide clarity on what exactly was the problem and we will check again.

Conditions are not working. For instance under "Job Info Change" ,
Separation and Leave of absence form populate even though the conditions require
    it not to.

I suggest you disable or remove/delete Condition #8 as it is faulty.

That field by default will be "Empty" when the form loads.

Because you still add condition rule when it is "empty" if will affect the other conditions after the form was already loaded. There is no point in time that the form will be reloaded unless the user will reset or restart the form.

You can check and test the form again after removing condition #8

Let us know how it goes.

Unfortunately, we do not provide phone support, but we will be happy to help you via this thread.

I have just tested your form and it seems like it behaves according to the conditions, example:

Please provide a clear example about what is not working, you can let us know which condition you're testing, the result you're getting and the expected result, replicating the issue will help us to find the cause.

Ok, the following is what I need the form to do:
-Job Info Change: remove Leave of Absence section & separation of
    Employment. Add the Submit portion
-Separation of Employment: remove Job Info Change & Leave of Absence
-One Time Payment: Add the Payment Section. Add the Submit portion
-Leave of Absence: Remove job info change, remove the Separation of
    Employment
-Work Eligibility: Remove job info change, Leave of Absence, Separation of
    Employment, Add the Submit portion.

The logic for your form seems very simply; however, reviewing your form I do not see conditions to behave as you want, for example, when the option "Job Info Change" is selected, nothing is hiding the fields, there is only one condition requiring some elements:

I would recommend you to check your current form conditions carefully and make sure you have configured them to trigger the actions you want, so for example, if for "Job Info Change"  you want to  remove Leave of Absence section & separation of Employment. Add the Submit portion, then you simply need to add a condition that hides the "Leave of Absence" and "separation of Employment" collapse sections as this will hide the entire fields in each section, you may implement the same logic for the rest, hiding each section according to the selection.

This guide will help you with the show/hide conditions: https://www.jotform.com/help/316-How-to-Show-or-Hide-Fields-Base-on-User-s-Answer

    Thanks.

    Hi -
I ended up just creating another form.
The logic it follows is working
correctly in this new form https://form.jotform.com/90577147744163.
I tested the form, but I did not receive an email similar to the old form
with a PDF confirmation, etc... I see where the setting is changed but I
don't know how to configure it. I need the email confirmation to work the
    same as this form https://www.jotform.com/build/82734278884169.
    I appreciate any assistance.
