My form has the wrong name on the tab when it opens

I cloned a form because I liked the format I had previously set up and even though I have changed it's name and saved it that way when you click the link and open it the tab says SACE the music no Anita Wardell Workshop - how do i change that??? i have looked through all the obvious edit places and they all say Anita Wardell Workshop. How do I alter this????
