    A few questions before we purchase:

    1.) is this form hosted by your company?

    2.) Can we edit CSS/HTML?

    3.) Can this forms info be sent to an email address once filled out?

    is this form hosted by your company?

    Yes. All the forms are hosted by JotForm and they can be accessed by using the direct link or you may use a custom URL or custom domain as well.

    Can we edit CSS/HTML?

    JotForm is an online form builder where no coding is required and the forms can be built using intuitive drag and drop method. However, if you want to customize the form by injecting custom codes you may inject the CSS code directly from the form builder for classic layout forms. If you still need to customize the form by adding custom codes, you may download the full source code of the form to make changes and host it on your server.

    Can this forms info be sent to an email address once filled out?

    Yes. You may setup notification (to you) and autoresponders (to the user) on the form to send the emails.
