They probably ceased their form service. I see you are using a form from Tectile. If you are looking to switch form builder, we can help you here.

The form on the website can be imported to Jotform. That way, you do not need to re-create it. The form can be formatted after and you can choose a design from our themes gallery.

The actual form URL is this one http://globalinsureall.com/questions-bilingual.htm?f=50&iframe=yes.

The result would be like this in our form builder https://form.jotform.com/90575850042961

My focus is not webdev, and I set all this up with MS Frontpage (yes it is no longer supported)
but I need to maintain all my sites myself as my insurers change links all the time

What would you charge me to do the simple form, and with anti-spam features like reCatptcha .....
what would be the annual fee?

I am AARP member age 60+

You can use a free account to build your form and have access to all the features, widgets, apps, integrations. You would need to upgrade your account in case you need to increase your account limits, for example, if you exceed the submissions limit.

You will find the details about the limits related to each account following this guide: https://www.jotform.com/help/408-Understanding-Your-Account-Usage-and-Limits

This guide will help you with the details about our pricing: https://www.jotform.com/pricing
