State needs substitution of AP-152

Good Afternoon - I am attempting to purchase a subscription for a state agency.  I requested the W-9 which I received but your agency is not set up in our system.  My AP department has requested that your agency fill out AP-152 form so we can set you up and I can make the purchase. However I am unable to attach here because it is s a pdf created by the state.  Is there an email address I can send this form too?
