Change title of my cloned form

Good morning I gave complete my first form by myself. I also clone if from my on my other forms. I am trying to publish but it coming up with the clone information https://form.jotform.com/90404181843150 its should say the Walton's Wedding Renewal Vows not Pomona Class reunion.

When a form cloned, it also its title and page title information clones. You have changed the form title. Great! You must also change the page title. 
Please look at this to change your page title.
