Embedded form is displayed twice in webpage.

I've add my jotform on my website. But when I add it, it come in 2 versions... I'd like to have just one. I'd like to avoid it to be in this kind of "window"

I am sorry for the trouble caused to you.

I did check your webpage and it seems you have added your form's embed code twice and that is the reason why you see two forms embedded in your webpage. Please check  the screenshot below:

I would suggest you to please remove the embed code you have added just before the footer div and that will solve your problem.
